Dental Insurance

We are committed to helping our patients maximize their dental benefits while simplifying the claims process. The following is information regarding dental insurance that every patient should be aware of.

Your insurance is a contract between you and your insurance company. We can never guarantee what your insurance plan will pay on your behalf. We will, however, do our best to help you understand your benefits and get an accurate estimate of what your insurance will cover. We will also file all the necessary claim forms for your primary insurance. If you have any specific questions or concerns about procedures and/or the amounts your plan covers, you can always contact your dental insurance carrier directly.

We are participating “In-Network” providers for the following plans:

  • Florida Combined Life / Blue Dental Choice PPO
  • BCBS Plus PPO (Note: this is not the Blue Dental PPO co-payment plan)
  • BCBS Federal Employees Program
  • Humana PPO
  • Aetna PPO
  • United Healthcare PPO
  • Assurant DHA PPO
  • Delta Dental (Note: Premier Plan only)

If you have an insurance plan that is not listed above, we can still file your insurance claims and accept assignment of benefits as long as you are able to see any dentist you wish. Many plans such as Eagles Benefits provide the same benefits “Out of Network” as they do “In-Network.”

Please remember that any estimate of insurance benefits we give you is only an estimate and is not a guarantee of insurance payment. You remain responsible for your entire balance regardless of insurance coverage or payment.

Payment Options

Payment is due at the time that treatment is rendered. You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment. We also accept all PPO dental insurance plans and are in-network for many insurance plans. We are happy to file all of your dental insurance claims for you. To discuss your payment options or your specific insurance coverage, please call us at (352) 372-0451. For your convenience, we are pleased to also accept Care Credit, which allows you to take advantage of flexible financing options.
